What to expect on your first
visit:
Your first visit with the physician will take about one
hour. Please come in at least 15 minutes before your
scheduled appointment so we can process your paperwork. If
time allows, you will receive a new patient packet in the
mail. Please fill out this paperwork completely and bring it
with you. You will also need to bring your insurance cards and
a photo identification with you.
